Abstract

Bilateral neck swellings are uncommon, accounting for only 8.3–10% of neck masses compared to their unilateral counterparts, and their diverse aetiology—ranging from infective lymphadenitis to salivary gland pathology and malignancy—often makes diagnosis challenging. We report a 45-year-old female with painless, slow-growing bilateral neck and retroauricular swellings of five years' duration, associated with recent fever and weight loss. Haematological work-up showed microcytic hypochromic anaemia with eosinophilia, and contrast-enhanced CT revealed bilateral cervical lymphadenopathy with a submental–submandibular soft-tissue lesion. FNAC suggested reactive lymphadenitis, and tuberculosis was ruled out. Following correction of anaemia, excisional biopsy of the left submandibular lymph node was performed, and histopathology confirmed Kimura's disease—a rare, benign, chronic inflammatory disorder that predominantly affects young Asian males. The patient remained asymptomatic with no recurrence at one-year follow-up. This case highlights the importance of considering Kimura's disease in the differential diagnosis of chronic bilateral neck swellings with eosinophilia, given its overlap with infective, inflammatory, and malignant conditions.
