A MULTI-DISCIPLINARY APPROACH IN THE MANAGEMENT OF ENDO-PERIO LESION: A CASE REPORT

Abstract

Management of poorly prognosed teeth with endo-perio lesion and grade II mobility is quite challenging. Surgical protocols like open flap debridement give a good treatment outcomes and filling the bone defects with biomaterials like bone grafts and collagen membranes help in predicting great amount of hard tissue healing with reduced epithelial penetration. Hence, the present case report shows management of a Grade II mobile tooth showing poor prognosis stabilized with orthodontic splints. Root canal therapy was performed and surgical treatment done with horizontal incision 3mm below the gingival margin and vertical incision at line angle of 35 exposing the intra bony defect (IBD). IBD was filled with Bio-Oss bone graft with collagen membrane and area was approximated with 4-0 silk sutures. One year post-surgically there was an improvement in clinical parameters with reduced mobility. Thus, present case report reconfirms the statement of “endo first and perio next” and also gives an idea of accessibility for thorough debridement of surgical defect and gives a ray of hope that with proper planning of treatment even a poorly prognosed teeth can be given a chance of survival.
