CBCT-Based Comparative Evaluation of Obturating Material Clindamycin Gel–Zinc Oxide with Endoflas-Clindamycin Gel in Primary Teeth: An In Vitro Study

Abstract

Background: Root canal treatment in primary teeth is challenging due to their complex canal morphology, which often results in void formation and compromised sealing. Enterococcus faecalis is a persistent pathogen in endodontic failures, and combining obturating materials with antimicrobial agents such as clindamycin may enhance treatment outcomes.

Aim: To evaluate the sealing ability and void formation of clindamycin gel combined with different obturating materials in primary root canal therapy.

Materials and Methods: An in vitro study was conducted using 18 extracted single-rooted primary teeth randomly allocated into three groups: Group I received zinc oxide eugenol (ZOE) with clindamycin gel, Group II received Endoflas with clindamycin gel, and Group III received ZOE alone. Obturation quality was assessed using cone-beam computed tomography (CBCT) by evaluating void number, size, type, location, and apical sealing. Statistical significance was set at p<0.05.

Results: The Endoflas-clindamycin group demonstrated significantly fewer voids than the other groups (p=0.019), with no voids larger than 2 mm and improved adaptation in the cervical and middle thirds. All three groups achieved complete apical sealing. Overall, the combination of Endoflas with clindamycin gel showed superior filling quality and sealing ability.

Conclusion: Endoflas combined with clindamycin gel provided superior obturation quality by minimizing void formation while maintaining an effective apical seal. This novel combination may improve pediatric endodontic outcomes and reduce the risk of reinfection.
