Comparison of Alignment Efficiency and Periodontal Health Parameters between SmartArch wires and Heat-Activated CuNiTi wires in relieving crowding - A Randomized Controlled Trial

Objective: To compare SmartArch (S-A) archwires with heat-activated (Damon) NiTi archwires in terms of alignment efficiency and effect on periodontal health in subjects treated with fixed orthodontic mechanotherapy.
Materials and Methods: This randomized control trial comprised two groups: (a) S-A group and (b)Damon group, with 10 subjects in each group. Complete treatment records were taken, and intraoral scans were analyzed for improvement in Little’s Irregularity Index (LII), which was measured at 0 (T0), 4(T1), 12 (T2), and 24 (T3) weeks intervals. External apical root resorption(EARR), crestal bone loss and gingival index were assessed in the lower anterior teeth. The statistical analysis was performed using SPSS software version 26. Paired ‘t’ tests for intragroup comparison and independent ‘t’ tests for intergroup comparison for the assessed parameters were done. Repeated measures ANOVA was performed to determine the changes in LII at each time interval.
Results: The LII scores improved over time within the two groups (p-value <0.05). However, there was no difference in LII scores between the groups at any time interval(p-value>0.05). A significant change was observed in most of the transverse dimensions within the group. On inter-group comparison, no significant difference was observed (P value >0.05). On comparing EARR, Group B reported with more resorption compared to Group A in the mandibular canine region(p value = 0.05). On comparing crestal bone height changes within the groups, both groups showed an increase in crestal bone loss, which was statistically significant (p Value<0.05). On inter-group comparison, there was no statistically significant difference. Gingival index scores did not show significant changes in inter-group comparison.
Conclusion: Both S-A and Damon wires effectively levelled and aligned the dentition. The transverse dimensions showed a significant increase in both groups; however, on inter-group comparison, no difference was found. EARR and crestal bone loss were observed in both groups. However, this was not clinically relevant.
Limitations: Root resorption needs to be examined over a longer period of time.
